Add a custom image or PDF to a form, such as a site map, a traffic control diagram or a safe work method statement, so workers can open it from inside the form while they are filling it in.

Before you start

Step 1: Upload the image or PDF to Documents
- Go to Resources > Documents.
- Click + Add document.
- Enter a Name for the document. Workers will not see this name on the form, so use something you will recognise later.
- Set Document type to Attachment.
- Upload your image or PDF.
- JPEG files display best on the mobile app.
- Click Save.
Step 2: Copy the document link
- On the Documents page, click the ⋮ menu next to the document you just uploaded.
- Select Copy Link.
- Copy the link from the pop-up that opens.

Important You must copy the link from the pop-up window that opens. The address in your browser bar is not the document link and will not work on the form. |
Step 3: Add the link to your form
- Go to Field Data > Forms > Templates and open the form template you want to add the image to.
- On the question where the image should appear, tick the Weblink (URL) box.
- You can add a weblink to any question type. Heading works best, because it puts the image above the questions it relates to.
- Paste the link into the Weblink field.
- Click Save.

Result The next time a worker opens this form, the image or PDF link appears on the question you added it to. |

Things to know
- JPEG images display inside the form. Other file types, including PDFs, may require the worker to click the open icon to view the file.
- One weblink can be added per question.
- The link points at the document in Resources > Documents. If you archive or delete that document, the link on the form stops working.
- To swap the image, upload a new attachment document and paste the new link into the form. Editing the form is what updates what workers see.
- Large image files take longer to load on sites where reception is poor. Resize before uploading where you can.
- Adding a weblink does not change any form results that have already been submitted.
